Clients may self-refer for prenatal care with Community Birth Program.

Just call the clinic at 604-575-7275 to book an initial appointment. You do not need a referral from your doctor. If you have had blood work and/or ultrasound(s) in the current pregnancy, we ask that you sign a records release for us to obtain copies of those results.

For Referring Physicians

Please complete a referral form and fax it to 604-574-7290 with any current labs or ultrasounds. We will contact your patient to set up the first appointment and notify you when she has entered our care. Care will be transferred back to you after 6-8 weeks postpartum.
