Community Birth Program

How To Register

The Community Birth Program is open to all who plan to deliver out of Surrey Memorial Hospital or are planning a home birth in Surrey, North Delta, or White Rock. If you would like to learn more about the program, please read about our care and our team, then fill out our intake form below and we will call you back within 5-7 business days to set up your first appointment.

Your initial visit will be a 45-minute appointment with a midwife or a nurse practitioner. We will order prenatal bloodwork and any required ultrasounds, review your health history with you and discuss what to expect during your care with Community Birth.

Please take care in filling out this form as accurately as possible; it will help us determine your care needs in advance of your visit.

UPDATE JANUARY 2024

WE ARE RECEIVING A HIGH VOLUME OF APPLICATIONS. IF WE ARE UNABLE TO SEE YOU WITHIN 2-3 WEEKS OF YOUR INTAKE FORM SUBMISSION, PLEASE BOOK IN WITH YOUR FAMILY DOCTOR, NURSE PRACTITIONER OR VIST A WALK-IN CLINIC TO GET PRENATAL BLOODWORK AND A DATING ULTRASOUND SCHEDULED AS IT MAY TAKE US UP TO 7+ DAYS OR MORE TO RESPOND.

MAKE SURE TO HAVE THE RESULTS COPIED TO COMMUNITY BIRTH PROGRAM TO PREVENT DELAYS IN OUR OFFICE RECEIVING YOUR IMPORTANT PRENATAL RESULTS. You are welcome to print the following Prenatal blood work and dating ultrasound requisitions for the care provider you see to order so results are copied to us before your initial visit. You will need to continue care with your provider until you have your first appointment with our clinic.
